An applicant can receive an application form for a visa free of charge at the Embassy or can download it from the web-site of the Embassy (click here in *pdf format in *doc format) or from the web-site of the Ministry of Foreign Affairs of the Republic of Belarus http://www.mfa.gov.by/en/consular/applications . The filled application form has to be legible and to be signed personally by the applicant (specimen of the filled application form can be downloaded here).
