The CTG represents a new and separate funding stream. This is a separate FOA and not intended as a direct follow-up to any existing funding. A list of CDC chronic disease and health promotion topics, as well as profiles for each state including fact sheets, funding, legislation, reports, state-based programs and projects, statistics, and data can be found on the CDC Chronic Disease Prevention and Health Promotion Web site.
