The 2004-2005 Canadian Hospice Palliative Care Association Annual Report describes their directory that does just this. It went online in 2002, and contains the contact information for nearly 500 hospice palliative care programs and services. This has “enabled broad access” to hospices in every province and territory in Canada. The site and directory are both available in English and French.
