Where can I execute an Advance Directive such as a Living Will and a Designation of Health Care Surrogate?
At Jackson Memorial Hospital, Advance Directive forms are available in the Admitting Office, or you may request them from your physician or social worker. You may use our forms and select the choices that suit your tastes. You may change any of the words you wish and add any further instructions. Or you may write down your wishes or the name of the person you chose to make decisions for you. You also may bring us your own forms, and they will be made a part of your medical record.
