Where are the hard copies of the deployment health screening forms filed in the medical record?
According to BUMED (M3B12), Patient Administration and Tricare Operations, completed hard copy DD 2795, DD 2796, and DD 2900 forms should be filed in Part 3 of the Navy-Marine Corps medical record. This is the same section that holds History and Physical exams (H & Ps) and Overseas Screening forms. Guidance on the organization of the hard copy medical record can by found in Chapter 16 of the MANMED, however, please be advised that this chapter is undergoing review and updating at present.
