Most forms on this national service are accessed through a state-specific legal aid website. For a list of such sites, visit www.LawHelp.org. Not all states yet have forms available. As an example, LawHelp California is the state-wide web site for California. There are a few links to HotDocs forms on this site, created by legal services programs in California. To see one that runs a name change template, click on “Families and Kids,” then click on “Name Change,” then select any city or county. You’ll see a “Name Change” page; click on the first link “Are you an Adult Changing Your Name? Start Here!”. That will bring you to this site, where you can log in or go directly to the form.
