We don’t have those forms. Child custody is different for every family, so it would be difficult to have one form on line that would work for everyone. You will have to create a petition to modify support on your own and file it with your county clerk. Your petition should look like a court document, so you should use one of the court forms you already have relating to the child(ren), and make yours look like it with the appropriate names, court and cause numbers. You should explain to the court in short, simple sentences what the situation is right now and how you would like for it to be. You must also explain that the change in custody that you suggest is “in the best interest of the child(ren)” because that is the same standard that the court will use to determine if the change is appropriate.
