A “Homeowner Improvement Request Form” can be found on the website under Homeowner Association and then under the tab for “Forms”. This is a PDF document that can be printed out, then completed and mailed to: Canyon Parke Estates HOA, Attn: Board of Directors, P.O. Box 1716, Helotes, TX 78023.
