The last race that was red-flagged was Malaysia 2009, but it was stopped due to a torrential downpour and not because of a crash. Qualifying and practice sessions get red-flagged “all the time”, pretty much whenever there would be a safety car if it was a race. The most recent Grand Prix at Hockenheim had one red flag during qualifying when Tonio Liuzzi crashed, and the 2009 Japanese Grand Prix had as much as four red flags in its qualifying session.
