This is a hardware fault and usually requires the replacement of the complete iPAQ / Axim glass touch screen. Under normal circumstances the touch screen wears out through natural use, and the ability for the screen to accurately read the input from the stylus fails and causes either spiking on handwriting or complete dead spots where you are unable to write on the screen.
