When submitting items to WIRB for review, is the use of the submission form required?
WIRB requires that a completed initial review submission form be submitted with each initial review request and change of principal investigator request. The cover page lists the items required in the submission packet Also posted on the website is a Change in Research and Subject Recruitment Submission Form.
