When must evidence of insurability be submitted to obtain coverage?
Evidence of insurability is required if you or your dependents: • Enroll more than 31 days after the date you are eligible for coverage • Voluntarily cancelled your insurance and are re-applying • Apply after your coverage ended because you did not pay the required contribution • Have not met a previous evidence of insurability requirement to become insured • Apply for coverage exceeding the guaranteed issue amount
