According to Section 440.491(5)(c), Florida Statutes, a carrier shall report its voluntary service activity to the Department of Education in the manner prescribed. This includes medical care coordination, reemployment services, and reemployment assessments. Rule 6A-22.010(9)(d) requires that the carrier file the form DWC-21 within 30 days of payment or partial payment of the form DWC-21.
