However, a formal specific written risk assessment is required in the following circumstances: • for all hazard severity level 4 chemicals • for all activities that have been judged to present a high risk of exposure i.e. for those activities where an exposure to a substance is considered likely • for all activities that have been judged to present a high risk of fire or explosion Two risk assessment forms are available: • ‘COSHH’ form for substances presenting a risk to health • ‘DSEAR’ form for ‘dangerous substances’ presenting risk to safety from e.g. flammable or explosive hazardous properties of a substance The risk assessment form must be approved by the Supervisor / Group Leader before work begins.
