An extension site can ONLY be initiated by programs with FULL APPROVAL STATUS. Programs should submit a new program notification form located with Guideline 3.1.2.a. Initiating or Reactivating an Extension Nursing Education Program to Board staff 4 months prior to implementation. Board staff may request additional information.
