No. A private school is only required to file an Affidavit each year it is operating. However, it is helpful to the CDE to receive a notice regarding the closure so the school can be removed from the mailing list. Please direct a closure notice stating the name and address of the school by mail to: Elementary Education Office – Private Schools California Department of Education 1430 N Street, Suite 4309 Sacramento, CA 95814-5901 By fax to: 916-319-0126; or by e-mail at privateschools@cde.ca.gov.
