Section 8.16.d. of the Provider Manual states that the transferring support coordination provider must ensure that a transfer summary and a copy of the support coordination record for at least the previous one (1) year time period be made available for transfer to the receiving support coordination provider. The service recipient can request additional records as needed to provide adequate historical information. Section 8.8.of the provider manual does not list support notes as being a part of the support coordination record. Support coordination monthly reviews that are required should contain adequate documentation of all funded services that were provided by the transferring support coordination provider.
