They are really synonomous. A rational or informed decision is based on knowing enough facts to be able to reach a conclusion about a course of action where the consequences can be foreseen or understood.

I think a rational decision is based on logic; an informed decision is made after considering certain facts and information (and may not necessarily seem logical)
