Health Advisors provide information and support for the following health issues: • Nutrition • High Blood Pressure • High Cholesterol • Pre-diabetes and Prevention • Physical activity/exercise • Stress • Weight Management • Tobacco CessationConcordia Health Plan members are selected for the Health Advisor program based on outcomes from the online Health Assessment (HA), or they may self-enroll, or they may be referred through other CareAllies programs.
