• Blank Admission Med Rec Form can be used for inpatients and outpatients • Inpatients: Med Rec Form can be printed from IBEX and/or Logician (if data exists) NOTE: IF printed from these sources this is oNLY a starting point, MUST be reviewed with patient • Outpatients: ANY qualified clinical staff member can review form with patient to determine if there are contraindications or interactions to treatment and sign Medications reconciled at admission and date/time. • After the procedure is complete, and prior to discharge, the clinician performing the review reviews the patients Admission Med Rec Form and any medications received during the encounter and reconciles the medications for discharge by circling C for continue, DC for discontinue or N for new. The clinician completing the reconciliation signs Medications reconciled at discharge and instructions provided to patient and dates/times.
