ALL or AML can be like that, and some very severe asthma can also be life-threatening if not treated (though it’s usually not asymptomatic if it’s that bad, even with treatment). Heart failure is another good one, diabetes can cause coma and death in a few days if you stop taking insulin.

Cancer can kill you faster without chemotherapy. HIV can kill you faster without drugs. I don’t know about immediate death, though. You could make up an illness for your story.
