Ans. If you have lost your Passport, you have to fill up Passport Application Form and enclose written statement stating how the passport was misplaced / lost / stolen, First Information Report (FIR) from Mauritian Police authorities, copy of valid work / occupation permit, copy of valid residence permit, photocopy of the lost passport, four recent photographs and fee of Rs.4375/-.
