What would happen if an Iranian warship shot down a US airliner over US waters?
Since no Iranian warship has any business being in US waters or firing anything it would clearly be an act of aggression. Now if we are discussing a war zone where hostile aircraft are operating as well as commercial airliners, if they claimed it was done in error and offered an apology and restitution, then it would treated as a mistake.
