This must be submitted to the workers compensation insurance adjuster for payment of mileage. The injured worker is entitled to receive 66.67% of his average weekly wages for each week off work. The worker must be off at least five days before being entitled to any payment. There is a limitation of $292.86 maximum weekly benefit (as of 1/1/99).
