The name is/was Forkosh Memorial Hospital. The son of the founder was Dr. David Forkosh, his (David’s) mother’s name was Bertha. I believe the correct address is/was 2544 W. Montrose Ave. To the left of the entrance was a sculpture made from chrome car bumpers called the “Healing Hand”.
