What types of correspondence will PCP’s receive?
• PCP’s will receive an introductory letter announcing the program. • You will also receive a letter when one of your patients agree to participate in the HMP. The nurse care manager will send you a list of items that will be addressed in the care plan as well as a request for your input on those items. • You might also receive a letter from the OU College of Pharmacy if a potential problem is identified on the members’ medication list. • The nurse care manager may also contact you to request records such as lab values, etc. • You will also receive periodic letters from your patients’ nurse care manager that will list goals, achievements and update you on their overall health status.
