The Plan is a self-funded health plan, administered for the exclusive benefit of eligible Plan participants. The ASEA Health Trust uses a PPO – a Preferred Provider Organization – for inpatient and outpatient hospital expenses, and for physical therapy services received in the Municipality of Anchorage. PPO provisions do not apply to physician services, so you can receive care from any licensed physician, in or outside Anchorage, and receive the same level of benefits. In ASEA’s Plan, you always have the freedom to receive care at the hospital or physical therapy office of your choice anytime you need care. If you receive care in the Municipality of Anchorage, you will, however, receive a higher level of benefits when you are cared for at an ASEA PPO facility, which is Alaska Regional Hospital or at Chugach Physical Therapy. If you receive care from a hospital or physical therapist in Anchorage that is not in the PPO, you will pay more out-of-pocket. For services other than inpatient/o
