What type of forms can DAR recommend for tracking individual vials and usage of controlled substances?
The Drug Inventory Form tracks individual vials based on drug type, delivery date, lot numbers and expiration dates. DAR has a system in place to issue a unique number for each vial as it is received from the distributor. A sample form is available: Drug Inventory Form.
