The documentation of participation in a seminar or workshop-type CEU activity must include a certificate or other verification of attendance which includes: 1.The name of the licensee participating in the activity; 2.The title of the activity; 3.The sponsoring agency, instructor, or trainer for the activity; 4.The date(s) of the activity; 5.The duration or length, in hours, of the activity; and 6.The signatures of a representative of the sponsoring agency, the instructor, or the trainer. If the title of the activity listed on the verification of attendance is not indicative of the content of the activity, or may not be sufficient for the Board to determine whether the activity will satisfy the requirement, you should keep a copy of the syllabus or course content to submit with your documentation especially if you are requesting credit for a part of a broader presentation.
