You have to figure out what the underlying cause is first. Could be something like thyroid problem (medication), bacterial or fungal infection (medication), may be something like demodectic mange AKA demodex (interceptor, medication if secondary bacterial infection which is usually cause of smell and possibly immunoregulin to help immune system) and if all the sudden occurring in an older dog with no history or generalized you may want to look for an underlying immune disorder (depends on problem but often medication) for that.
