The completed worksheet should be provided in the Substantiating Records section using either the upload or fax capability. In addition, a screenshot copy of the DTS Reservation Module or CTO-provided fare cost should be included. Finally, be sure that the other cost factors (the CTO fee and taxi or public transportation costs to and from the airport(s)) are reasonable.
