Each semester, GW’s certifying officers will need a copy of your Vocational Rehabilitation Form (22-1905 “Authorization in Certification of Entrance or Re-entrance into Rehabilitation and Re-certification of Status”) from your rehabilitation counselor. Please fax or hand deliver this document, and then we can prepare the voucher for you.
