What services are provided by Transitional Case Management?
Transitional Case Management prepares alcohol/drug abusers to receive chemical dependency treatment and health care. It provides outreach, short-term counseling, information and referral to treatment. The goal of Transitional Case Management is to help clients overcome barriers to treatment, such as medical or mental health issues, social service or legal problems, or housing difficulties. HIV prevention interventions are also provided. Who is eligible to receive help from the Transitional Case Management staff? Anyone who recognizes that he/she has a chemical dependency problem, and is unsure about how to find help, can benefit from Transitional Case Management services. Is there a charge for Transitional Case Management services? All services provided by Transitional Case Management are free of charge.
