What Services are Provided by Horizon Hospice?
Our Medical Director is the leader of our clinical team. We provide a professional and caring Clinical Staff to do intermittent visits as determined by you, your Doctor, and our team to assess your needs and teach and or provide care as appropriate within hospice guidelines. • Registered Nurse • Home Health Aide and/or Homemaker • Social Services and Spiritual Support • Therapies & Dietary services, if needed • Volunteers On call 24/7 One of our RNs is on call 24 hours a day, 7 days per week to answer any questions you or your caregivers have and make extra visits as needed. Bereavement Services We follow our families with grief support, as they wish, for 13 months after the loss of a loved one. Medication and Medical Equipment and supplies We provide and pay for any medication and or equipment / supplies needed related to your end-of-life diagnosis that is determined necessary by your Dr. and our team, for your comfort.
