What Services Are Offered by Assisted Home Care?
Assisted Home Care provides the short-term intermittent nursing or therapy visits prescribed by your physician. Care can be arranged for infants through adults. Care is typically initiated following a hospitalization (or short-term stay in a skilled nursing facility). With hospital stays shorter and shorter, home health helps transition you back to health in the comfort and security of your own home. Your physician can also prescribe home health care if there is a recent change in your health status or an exacerbation of an existing condition. To qualify for home health visits you must be homebound – meaning it takes a taxing effort to leave the home. Care must be restorative or rehabilitative, not custodial. Intermittent visits involve a nurse or therapist coming to your home to perform a clinical procedure or to guide you through a therapy session. The care must be medically necessary and involve the services of a licensed professional.
