The approach to an adolescent with “prediabetes” would be to get an oral glucose tolerance test with paired glucose and insulin levels at each time point. This would then define impaired glucose tolerance. I am making the assumption that this is an asymptomatic, obese adolescent. Once impaired glucose tolerance is confirmed, the treatment is weight reduction through diet, exercise and life style change. For severe insulin resistance with impaired glucose tolerance some people use Metformin, but it is not FDA approved for that indication. Children’s has a screening clinic for these patients. They are scheduled for the OGTT, a fasting lipid panel and see one of the diabetes Nurse Practitioners for an initial evaluation. If appropriate, following this screening evaluation, they may be scheduled to see one of the endocrinologists for further management decisions.
