The carrier shall, upon audit, verify the base wages paid and regular hours worked used in the promulgation of the Policy Credit Factor are accurate. If discrepancies are found, the carrier shall instruct the employer in writing (WCPAP Carrier Audit Form) and copy the WCRB to complete and submit a WCPAP Carrier Audit Form. The carrier will defer its audit for no more than thirty (30) days, giving the employer an opportunity to reapply. The WCRB will recalculate and release a revised policy credit worksheet. If the carrier has not received a revised Carrier Audit Form from the employer or Policy Credit Worksheet from the WCRB within thirty days; then a written request to withdraw the policy credit must be sent to the WCRB.
