Adults must present a photo I.D. (preferably a driver’s license), a social security number, a photograph of the child being collected and payment for the DNA test and the sample collection fee.

Adults must present a photo I.D. (preferably a driver’s license), a social security number and payment for the DNA test. The legal guardian of the child must authorize the test on behalf of the child. Minor tested parties also need legal guardian authorization.
