Three questions need to be included in the evaluation forms for all QA&CPD Program –approved activities. See the May 2007 Provider newsletter for information regarding the mandatory questions. Education activity representatives are free to include whatever other content they wish on an evaluation form (as long as the three mandatory questions are present).
