What needs to be done when LGC HealthTrust-covered subscribers get divorced or legally separated?

A subscriber who is either getting divorced or legally separated needs to complete LGC HealthTrust’s Medical and/or Dental Application and Change Form (HT 035) by checking off “Divorce/Legal Separation” and indicating the actual date of the divorce or legal separation in the “Reason for Completing Form” box. Additionally, the subscriber must complete and submit a Notice of Divorce of Legal Separation (HT-008) form. Both forms are downloadable from our Forms Web page. In some cases, it may be necessary for LGC HealthTrust to request a copy of the related court decree.
