What name should the applicant put on her WHP application?
Medicaid policy requires that applicants be certified for benefits with the names on file with the Social Security Administration (SSA), which are the names printed on their current Social Security card. However, WHP does not require that applicants submit a copy of their Social Security cards, and a copy of a Social Security Card cannot be used as proof of citizenship or identity. Some women may change their names legally because of marriage, divorce or other reasons, but still not take the additional step of going to a local SSA office to report the name change.
