What must be included in correspondence sent to the Division of Certification & Preparation?
All correspondence sent to Louisiana Teacher Certification and Higher Education should include the following: 1. Full name and date of birth 2. Social Security Number 3. Certificate number (if known) 4. Degree(s) earned, when and where 5. Mailing address 6. Telephone number and email address where he/she can be reached. With all application requests, a certification fee and a Professional Conduct Form should also be included.
