Each company complete the MU1 Form for the company. Each direct or indirect owner of 10% of the company must complete an MU2 Form. In addition, the officers of the company such as the CEO, President, CFO, etc., must also complete an MU2 Form as well. If the company has branches that will conduct Oregon residential mortgage loans, the MU3 Form must be completed for each branch. Finally, the branch supervisor must complete an MU2 Form.
