What monitoring and/or reporting requirements must be adhered to?
The University requires that you submit an Annual Progress Report every 12 months after the initial approval for your project (if the project is continuing) and a Final Report once the project has been completed or if the project is discontinued for any reason. The same form is used for both purposes (progress and final). Although the Secretary will send you a reminder letter when your annual or final report is due, the responsibility lies with the Chief Investigator to ensure these progress and final report. The form must be signed by you and sent to the Secretary, Research Office, Level 3, Research HUB, Building C5C, Macquarie University NSW 2109. Progress & Final Report Forms NOTE: Three years after the original approval date researchers who wish their project to continue must submit a new application to the Biosafety Committee. The Committee is not able to approve the continuation of a project after three years on the strength of a progress report only.
