What medications are listed on the over-the-counter parental permission form?
Tylenol or generic for pain per package dose Advil/Motrin or generic for pain per package dose Antacid medication per package dose Calamine lotion or 1% anti-itch cream/spray Sunscreen Over-the-counter antibiotic ointment Cough drops/throat lozenges All of the above medications will only be administered as needed and once a day. These medications will not be sent on field trips. If more frequent administration is needed, please have your physician complete a "Physician’s Authorization for Medication at School" form.
