• If you wish to release your medical record to anyone, you must first sign a release form, which is available at Student Health Center (SHC). • The SHC Medical Records Department is responsible for receiving, filing, and securing your immunization records, medical records, and telephone/fax messages regarding your care. • Every student is required to complete the “Immunization & Medical History Form”. • You can avoid long lines and delays in registration by sending your completed form to the Student Health Center as early as possible. Forms can be mailed to: UNCW Student Health Center, 601 S. College Rd., Wilmington, NC 28403. Forms can be faxed to the SHC at 910.962.4130. • Download the Immunization & Medical History Form • Please call 910.962.3280 for questions or concerns about medical records or immunization requirements, or visit the Student Health Services (SHS) website. • Students entering college must present a “Certificate of Immunization” within thirty days of registration.
