Although the Affidavit in Appendix C of Resolution T-17143 covers this, we encourage applicants to provide a separate attestation to this similar to the affidavit using more specific language, and any other information that demonstrates the funding source. If the applicant provides a separate attestation re: matching funds, this can be included with item 12 of the checklist.
