What makes the FormFox form different?
The form used in the FormFox system has several paradigm–shifting attributes: • It´s universal. This means that it can be used for any type of collection and for any participating lab! • It´s only one page. The donor information, the donor receipt and all the necessary bar–coded sample labels – it´s all there on a single 8 ½ x 11 sheet! • It´s unique. Each individual form is pre-printed with a series of barcodes all containing the same code. This distinctive relationship between the various parts of the form makes it possible to link them to each other (even forensically linkable if you compare the ink).
