The Patient Care Area Pharmacists (PCAPs) are decentralized and have prescriptive authority in select antibiotic dosing (i.e. vancomycin, aminoglycosides), adult renal dosing, TPA for stroke, total parenteral nutrition, and anemia management. In the ambulatory care setting, the PCAPs have protocol authority in both the Anticoagulation and Anemia clinics, and in both our Family Practice Clinics.
