What kind of supporting documents need to be submitted with the claim form?
A statement from the provider and/or explanation of benefits (EOB) from the insurance carrier which shows the name of the provider, type of service provided, who the service was for, the date the service was provided and the amount of the service. Canceled checks are not acceptable as sufficient supporting documentation. Cash register receipts are not acceptable except when submitting claims for over-the-counter contact lens supplies.
